How it works

Exclusion criteria

GLP1 medications

  • All GLP-1 receptor agonists are contraindicated in patients with hypersensitivity to the drug.
  • There is an increased risk of fatal hemorrhagic and necrotizing pancreatitis using any GLP-1 analog.
  • Semaglutide and tirzepatide are contraindicated in patients with a family history of medullary thyroid cancer or patients with multiple endocrine neoplasia syndrome type 2 (MEN).
  • GLP-1 receptor agonists should be avoided in gastroparesis or inflammatory bowel disorders.

     

    Inclusion criteria for weight loss medications for adult:

    • Age ranges from 18 to 75 years old.
    • Have a body mass index (BMI) of 30 or higher.
    • Have a BMI of 27 or higher and have at least one obesity-related condition, such as high blood pressure or type 2 diabetes.
    • Have not lost at least 5% of their total body weight in three to six months with lifestyle changes alone.

       

      Exclusion criteria

      Testosterone replacement therapy (TRT)

      • Prostate cancer.
      • Men with prostate cancer or a palpable undiagnosed prostate nodule should not take TRT. Men who are at high risk for prostate cancer or have severe urinary symptoms from prostate enlargement should also approach TRT with caution.
      • Heart disease
      • Men with uncontrolled heart failure, a history of myocardial infarction or cerebrovascular accident within the past six months, or multiple risk factors for heart problems should not take TRT. Patients should also not be enrolled within four months after an acute coronary syndrome or stroke.
      • Breast cancer.
      • Untreated obstructive sleep apnea.
      • Hematocrit over 50%.
      • Men planning fertility.
      • Thrombophilia.
      • Severe lower urinary tract symptoms.

      What is testosterone?

      Testosterone is a hormone made by the body that is responsible for the normal growth and development of the male sex organs and for the maintenance of other sexual characteristics. In men, testosterone is produced in the testes, the reproductive glands that also produce sperm. The amount of testosterone produced by the testes is regulated by the hypothalamus and the pituitary gland. Testosterone can affect the following:

      • Growth and maturation of prostate and other male sex organs
      • Development of male hair distribution such as facial hair
      • Changes in body muscle mass and strength and fat distribution
      • Sex drive and sexual function
      • Mood and energy level
      • Bone strength
      What are “normal” levels of testosterone?

      In healthy men, testosterone levels between 350 ng/dL and 1000 ng/dL are referred to as “normal.” Normal physiology allows the brain and the testes to work together to keep testosterone in this range. The brain produces “signal” hormones that are responsible for stimulating testosterone production. These signals are periodically released, which in turn stimulates testosterone production. When levels of testosterone are in an acceptable range, the production of the signal hormones are slowed, which in turn allows the testosterone levels to decrease.

      What is hypogonadism?

      Hypogonadism, in a male, refers to a decrease in either testosterone production, sperm production, or both. The medical professionals at Low T Center address the testosterone aspect of this condition. Decreased sperm production requires a careful, thorough evaluation by a fellowship-trained specialist.

      How does Low T Center determine if I have hypogonadism (low testosterone)?

      Testosterone treatment is the practice of diagnosing hypogonadism (low testosterone) through a combination of symptoms and testosterone blood values below the lower limit of normal and treating with testosterone injections, gels, patches, surgically implanted pellets, or oral applications. Low T Center uses injection therapy to treat low testosterone in hypogonadal men.

      What are the benefits of testosterone replacement therapy?

      Benefits of testosterone replacement therapy vary based upon the pre-therapy symptoms and other factors, but they can include the following:

      What is low testosterone treatment?

      What are the risks of testosterone replacement therapy?

      Some patients could experience one or more of the following side effects from testosterone replacement therapy:

      • Increase in red blood cells.This can be beneficial if you have anemia. However, it can be potentially dangerous because an increase in red blood cells can lead to blood clots, heart attack, or stroke.
      • Prostate effects.If you have an enlarged prostate, testosterone may worsen your symptoms, particularly if you are more than 50 years of age. If you have a history of prostate cancer, you cannot receive testosterone therapy without prior clearance from the urologist who is overseeing your care.
      • Skin reactions.Acne, oily skin, increased body hair, and flushing have been reported. These side effects are not very common, but if they occur, often they are transient.
      • Testosterone therapy down regulates production of a man’s sperm. Be upfront with your medical provider about your desire for children, and be sure to discuss the situation with your spouse or partner, if appropriate.

      Fluid retention. Although uncommon, you must use caution if you have a history of heart failure or kidney disease.

      In what cases should I avoid testosterone replacement therapy?

      Only your physician can fully answer this question; however, in general, testosterone replacement therapy is not recommended for, or should be avoided by, patients with the following conditions:

      • Breast or prostate cancer
      • A palpable prostate nodule
      • Erythrocytosis
      • Hyperviscosity
      • Severe benign prostatic hyperplasia symptoms (AUA prostate symptom score > 19)
      • Uncontrolled severe heart failure
      • Unexplained PSA elevation
      • Severe lower urinary tract symptoms associated with benign prostatic hypertrophy

      Unstable severe congestive heart failure (class III or IV)

      What are the advantages of testosterone injections versus other replacement methods such as patches or gels?

      The proper method of testosterone delivery for you is a matter for your physician to determine; however, many find that some gels tend to be messy and less convenient than other treatment modalities. In addition, there can be a risk of unintentional transmittal to children or others with whom you may have physical contact.

      Testosterone patches can create a significant rash at the site of application. They also don’t stick well, especially during the summer months.

      Experience has shown that gels and patches may require dosage adjustments to obtain medically appropriate blood concentrations, and some patients may never absorb enough testosterone from gels or patches to improve symptoms. These modalities often have a higher conversion to less desirable hormones in the process of transfer through the skin.

      Does Staminax Low T and Weight Loss Clinic accept insurance payments?

      No, Staminax Low T and Weight Loss Clinic does not accept insurance payments. All services must be paid for out-of-pocket. We offer a variety of payment options to make our services accessible to everyone.

      How Does GLP-1 Medications Work?

      They slow gastric emptying (how quickly food leaves the stomach after meals) and balance insulin production. As a result, you feel full for longer, promoting your ability to lose weight.

      What are the goals of GLP-1 therapy?

      To lessen appetite, cravings, and help you control portion sizes to facilitate up to the loss of 1-2 lbs/week, while minimizing unfavorable side effects.

      What are the side effects?

      The most common side effects are nausea, vomiting, stomach upset, diarrhea, tiredness, dizziness, or constipation. Initial side effects usually lessens as you continue to use this medication. Rarer side effects found on the product insert include: Dizziness, fatigue, gastrointestinal, headache, and stomach pain and/or distension. We try to minimize these side effects by beginning on a lower dose and slowly increasing the dosage until we find the right amount to take to facilitate your weight loss.

      Who is a good candidate for GLP-1 treatment?

      The ideal patient has a high BMI and has been unsuccessful with dieting and non medical weight programs.

      Who is not a good candidate for GLP-1 treatment?

      Patients who have a history of diabetes (these drugs could make your blood sugar too low), who have a history of medullary thyroid cancer or pancreatitis, and patients who have a personal or family history of multiple endocrine neoplasia syndrome type 2.

      Do I need to follow a particular diet while taking GLP-1 agnonists?

      There are no food restrictions with these medications, but your results, as well as your overall wellness, will be enhanced with an active lifestyle and a healthy diet.

      I have a history of a certain medical condition. Am I eligible for GLP1?

      My relative has medullary thyroid cancer? Am I eligible?

      A first-degree relative with medullary thyroid cancer is a contraindication. A strong, persistent history in multi-generations outside of a first-degree relative is also a contraindication. Please discuss with the medical provider all relatives diagnosed with medullary thyroid cancer. To date, MTC after GLP-1 administration has been observed only in rodents and not in humans.

      I suffer from gastrointestinal issues. Can I begin GLP-1 medications?

      Yes. Since GLP-1 slows food transit through the intestines, you may experience nausea, acid reflux, constipation, or diarrhea. We have medication at Options to help with this and can also prescribe medications to your local pharmacy. NOTE: GLP-1 medication may trigger pancreatitis, gallbladder attacks or gastroparesis. Discuss with your medical provider if you have a history of these conditions.

      Is GLP-1 safe during breastfeeding?

      While there have been no observable detrimental side effects noted in nursing mothers taking GLP-1 for other reasons (i.e. diabetes), there has not been enough data or studies to determine if there are long term effects on breastfeeding mothers or infants. Additionally, milk production requires an additional 500 calories per day. Currently, it is not recommended to breastfeed while taking GLP1 medications.

      Is GLP-1 safe while I am trying to conceive?

      If you have discussed fertility plans or treatments with your doctor, it is recommended to stop GLP-1 medications at least 2 months prior to TTC. Please speak with your physician to determine if a longer washout period is recommended.

      I have needle phobia, and I am not comfortable injecting myself. Can I still take GLP-1?

      Injection training handouts and videos are available to you. You may also request an injection training appointment with our Medical Provider Assistant who can do an in-person or telehealth video appointment to train you in the process.

      I have a history of psychiatric illness. Is it safe for me to begin treatment with GLP-1?

      Limited data has indicated a small subset patients experience mood changes after the initiation of GLP-1. Please discuss with your medical provider if you do not feel like yourself. If you are having suicidal thoughts, stop the medication immediately and call your medical provider. The Suicide & Crisis Lifeline at 988 is available by phone or text 24/7 for immediate help.

      Do I need to stop my other diabetes medications while on GLP-1?

      Do not stop your diabetes medications right away. Talk to your primary care physician to titrate your medication doses down as your blood sugar and hemoglobin A1c improves.

      I was not home/left my package outside/forgot to retrieve my package with GLP-1 medication. I put it in the refrigerator as soon as I discovered it. Is it still good to use?

      All medications should be refrigerated when they are received. Guidance on specific medications is below:
      Semaglutide (PQ): Useful unrefrigerated for 3 days, up to 90 degrees F.
      Semaglutid (Empower): Useful unrefrigerated for 3 days, up to 140 degrees F from the time of shipping.
      Tirzepatide (Empower): Useful unrefrigerated for 3 days, up to 90 degrees F.

      I injected too much medication. What should I do?

      You may experience some GI upset or other side effects. Please call Options so we may determine the dose you took and walk you through a plan.

      My vial did not contain enough medication for the fourth dose, it spilled out or leaked from the syringe.

      Please call us so we may document what happened, walk through preventive measures, and potentially dispense another vial or syringe of medication.
